AMC was founded in 2002 by Nesim Bildirici who felt the need for personalized care management services while caring for his chronically ill father, Gabriel Bildirici.

AMC supplements care provided by physicians by monitoring patients on a day-to-day basis for any deterioration in their conditions. AMC's services combine innovative technologies with a human touch to ensure that a patient's care plan is producing the desired outcome.

AMC is the first disease management company to:
	work directly with providers, health plans, physicians and their patients
	integrate case management and disease management
	manage multiple conditions

Consequently, the company is accountable solely to each client and their family for producing results in terms of quality of life improvements.
