I already visit my doctor regularly. How would I benefit from daily health monitoring?
A daily record of your vital signs, symptoms and medication compliance can give you and your doctor a more objective, accurate picture of your health and your diagnosed conditions. The doctor will have a better sense of how closely you are following your care plan and whether medications prescribed are having the expected effects. This way, you can receive more optimal health care. In addition, if there is any deterioration detected in your  condition(s) between visits to the doctor, you will be contacted by a nurse to discuss a plan of action.

 

I already have a home attendant. How can I benefit from AMC's services?
Home attendants are not licensed to measure vital signs or dispense medication. AMC collects information about your health and analyzes it daily. We also provide 24X7 access to registered nurses. In addition, AMC coordinates services through a provider network to address clients' non medical needs. Therefore, home care agencies' services complement rather than overlap with AMC's.

Finally, AMC's monitoring devices can be used to track home attendants' compliance with their daily tasks.

 

Who monitors my progress once I get started with the service?
AMC's nurse triage staff monitors your daily data around the clock and will contact you and your caregiver if any abnormalities are detected. In addition, your personal health coach will be monitoring your compliance with your care plan as well as the care plan's effectiveness over time. This latter process may include additional home based nurse's assessments as needed.

 

What if I want my doctor to see my readings?
Copies of monthly reports can be mailed to your doctor at your request. In addition, the information can be made available to your doctor online, again, upon your request.

 

How will I know how to use the devices?
AMC's self-testing devices are very easy to use. AMC's installers will provide you with a quick tutorial.

 

Will there be any disruption to my telephone or Internet service? Will I need to have more than one telephone line to avoid disruption?
Your telephone and Internet services will experience no disruption due to AMC. AMC's devices wait until the lines are idle to transmit any information collected. There is also no need to install additional lines. The transmission of the information takes only a few seconds.

 

Are there any extra local or long-distance charges incurred when my information is transmitted through my phone line?
No. The information is sent to a toll-free number at no cost to you.

 

How can I obtain a summary of my data for a particular month or time period?
Every month AMC will mail you:

  • A monthly report, summarizing vital statistics and any other interactions occurred during the month, like, for example calls to the nurse triage service.
  • The latest version of your Personal Health Record encapsulating all of the medical information collected by AMC. You will be asked to update this information to include any changes during the month, e.g. new prescriptions.

You and your caregivers will also have the option of accessing this information online.

 

How safe is my personal information?
AMC is bound by HIPPA regulations that protect the privacy of your medical records. In addition, all information posted online is encrypted and password protected.

 

Can I take my measurements when I am traveling?
Most of AMC's self testing devices are light and highly portable. Any measurements you take while you are away from home will be stored and time-stamped by the device. The information will be sent to AMC as soon as you reconnect the device when you get home.

 

Are AMC's services covered by insurance or Medicare?
No, AMC's services are currently not covered by insurance or Medicare.

 

I am interested in trying AMC's services. What do I do next?
Plese download the Pre-Visit questionnaire from the "Contact Us" section. You can then fax the completed questionnaire to AMC at 718-998-9247 or mail it to Advanced Monitored Caregiving, 1729 East 12th Street, Brooklyn, NY 11229. We will then contact you to schedule an in-home nurse assessment.

 

How do I discontinue my AMC services?
Just call one of our Customer Service representatives and we will send an installer to retrieve the devices. There might be a minimum charge due to the early termination of the service.
