Assessment:

plan for each client. A registered nurse visits
each client at
home. The assessment will cover medical, social,

will be performed to evaluate changes in clients'

Daily Health Monitoring:

Each client is provided with the appropriate,
easy-to-use self-monitoring
devices as identified in the Personalized
Action Plan, which are installed by AMC in
the client's home. These devices are connected,
via a phone line, to AMC's central facility
where live, highly-skilled staff monitors
the client's data around the clock (e.g.,
vital signs, symptoms survey, medication compliance).
If any abnormalities are detected,
a registered nurse will contact the client
immediately to try to determine the origin
of the abnormality and suggest a plan of action.
In addition, the client or the caregiver can
access the data (which is updated in real
time) via the Internet. |

There
is a registered nurse available by phone and
ready to answer health-related questions for
AMC clients, 24 hours per day, 7 days a week.

Personal Health Record:

All information gathered by AMC relating to
each client is stored in a Personal Health
Record (PHR). The PHR includes data gathered
during the initial nurse assessment and subsequent
reassessments by a personal health coach as
well as vital statistics taken from the home-based
self-measuring devices.
The collected information is synthesized in
monthly reports and can enhance a physician's
ability to care for the client by summarizing
the client's day-to-day health
status between visits. The information is
also accessible to clients and their caregivers
through the Internet and can be made available
to doctors and emergency services on request
through AMC's call center. |

Personal Health Coach:

AMC provides each client with a Personal Health
Coach whose mission is to support the client
in the management
of his/her health. The Personal Health Coach
will provide dedicated phone consultations
to the client and, at the client's request,
will visit the client periodically. |

Quality of Life Services:

These services are designed to take care of
geriatric clients' non medical needs. They
cover a wide range of areas including social,
financial and home care. Recommendations are
made to each client on the basis of the personal
