Job Type: Full-time, Weekend

We are hiring for several Nurse Care Manager positions on both the East Coast and West Coast time zones. The Nurse Care Manager is responsible for the telephonic management of complex cases identified for the telehealth program. Remotely assesses patient health problems and needs, develops and implements nursing care plans, and maintains medical records. Active, current New York license in good standing is required. Would prefer candidates living in a compact state in the northeast time zone. Must have cardiac experience as well as knowledge of care management.

 

Essential Job Functions:

  • Conducts assessments and evaluates biometric and IVR data and trend analysis, and care planning for selected members/patients.
  • Collaborates with the member/patient, family, physician and all members of the healthcare team, internal and external to this organization.
  • Coordinates the delivery of high quality, cost-effective care based on a customized model of care supported by clinical practice guidelines.
  • Advocates for the member/patient and family among various sites to coordinate resource utilization.
  • Interacts with the member/patient, family, physician and healthcare team. Utilizes the case management process to set priorities, plan, organize, and implement interventions that are goal directed toward self-care/long term care outcomes, and the transition to stabilization or independent status.
  • Utilizes “Motivational Interviewing” techniques to influence member/patient behavioral changes to increase the level of compliance with customized care.
  • Encourages client/member/patient participation and compliance in the clinical program efforts.
  • Documents accurately and comprehensively based on the standards of practice and current organization policies.
  • Interacts and communicates, telephonically, using video, or via other technologies to be develop, striving for continuity and efficiency as the member/patient is managed along the continuum of care.
  • Understands fiscal accountability and its impact on the utilization of resources, proceeding to stabilization and self-care outcomes.
  • Evaluates care by problem solving, analyzing trends and variances and participating in the quality improvement program to enhance member/patient outcomes.
  • Performs care management responsibilities as per customer requirements; recognizes urgent situations and escalates issues appropriately.
 

Position Requirements:

  • Minimum of five (5) years broad clinical experience.
  • Minimum of three (3) years’ experience in home care, care coordination and transitions of care, case/disease management, telehealth or related specialty fields which may include Pulmonary, Endocrine, Cardiac, and other populations with chronic multiple morbidities.
  • Basic computer skills such as accessing the internet and navigating from one application to another.
  • Other proprietary technological proficiencies will be acquired on the job.
  • Nursing degree from a college or university. Appropriate experience may be considered in lieu of degree.
  • Flexible work hours, evenings and weekends, time scheduling, travel or on call work specific to the care management program.
 

*Registered Nurse License in New York State is required, prefer compact state license; license(s) must be active and in good standing.
*Requires ability to work effectively from remote home-based office.

AMC Health is seeking a Full Stack Software Engineer Lead to join our software engineering team. AMC Health is fundamentally a healthcare solution provider; we leverage cutting edge technology and data to play a positive role in promoting lifestyle and behavior changes by supporting patients on their journey to better health. The Full Stack Software Engineer Lead at AMC Health is responsible for the architecture and design of our core platform, for implementation of highly scalable, fault tolerant, and high performance distributed cloud systems, and for fostering a collaborative environment with fellow engineering and product development professionals. The position reports to AMC Health’s VP of Product Engineering.

 

Technologies and Disciplines:

Our platform consists of a full stack javascript environment, using a polyglot persistence architecture, and running in a distributed virtual private cloud at AWS. Some of our technologies:

  • Stack: Javascript, MEAN (Mongo, Express, Angular, Node JS)
  • Mobile: Android, iOS, Cordova, webRTC, Bluetooth
  • Data: REST and WebSocket API’s talking to Mongo and Cassandra
  • CI/CD: Jenkins, Chef, and AWS OpsWorks
 

Responsibilities:

  • Overall solution architecture, data modeling, high level design, low level design and non functional technical requirements
  • Write code, daily, for Node.js persistence layers and web services, and Angular.js single page applications and mobile solutions
  • Solve complex technical, scalability or performance challenges
  • Define and ensure compliance to software engineering standards across the engineering team
  • Evaluate technologies and prototype solutions
  • Mentor other engineers, conduct code and design reviews and be hands on to develop technical products
  • Consume, understand, maintain and enhance the automated software deployment pipeline
  • Work in a fast paced agile environment and be able to take a high level concept and work with different teams to make it a reality
 

Competencies / Requirements:

  • Has deep exposure to all aspects of modern web application architectures, visual design, and mobile web development;
  • Has experience configuring web servers (Apache/NGINX), application servers (Tomcat/Express/NodeJS), and common server components utilized for NodeJS web software development;
  • Understands and embraces the value of automated software deployment and release automation;
  • Has experience in contributing to open-source projects;
  • Has a BS degree or equivalent in Computer Science or related fields, or commensurate training and real-world experience.

The overarching function of an HSC is to support the clinical team by conducting administrative and clinical duties. The overall purpose of the Medical Assistant is to provide the necessary support to the clinical team, in order for the patients to be able to meet adherence to their program of care outlined by their physician.

 

Essential Job Functions:

  • Completes Welcome Call and Core Evaluation.
  • Locates and confirms MD office and Fax numbers
  • Follow-up on hospitalizations to alert TCM team when discharged
  • Supports clinical team by executing requests for information from clients and physician offices and coordinating services per their direction.
  • Encourages customer/member/patient participation and compliance in the clinical program efforts.
  • Makes referrals to, coordinates, and follows through on patient interventions as directed (transportation, housekeeping, home modifications, food, etc).
  • Follows up with program participants to ensure ordered services are initiated and meeting expectations.
  • Documents accurately and comprehensively based on current organization policies.
  • Initiates outbound correspondence (ex: phone/fax/email) to clients, customers and providers.
  • Accepts inbound calls, taking and delivering messages appropriately.
  • Maintains/ updates client and customer electronic records.

Experience/Education Requirements:

  • A degree from medical assisting programs accredited by the Commission on Accreditation of Allied Health Education Programs (CAAHEP) or the Accrediting Bureau of Health Education Schools (ABHES) is required.
  • 6 months experience performing general duties of a Medical Assistant in a health care setting
  • Excellent organizational, communication and customer service skills and the ability to utilize information Systems effectively (such as Microsoft Office Windows products, Outlook, etc.) is required.
  • Knowledge of medical terminology and previous experience with electronic health record systems.
  • Demonstrate credibility with physicians and maintain productive and collaborative relationships with all clinical staff.
  • Ability to maintain patient confidentiality.
  • Ability to work well with others in team oriented environment, work independently when needed, take initiative and use good judgment.
 

Licensure and /or Certification Requirements:

  • CMA (AAMA) certification for medical assisting is required
