Exempt Position

Primary Job Function:

The overarching function of the Team Lead is to support AMC’s people and processes directly responsible for retaining patients in the RPM program. The overall purpose of the TL is to ensure appropriate processes are defined, implemented and measured to positively impact patient care and clinical outcomes.


Essential Job Functions:

    • Support workflow analysis and redesign projects
    • Implement and monitor processes related to patient care and retention
    • Review, analyze and report on metrics related to patient care and retention, to all stakeholders
    • Ensure successful completion of SLAs and other client commitments
    • People management
    • Ensure use of “Motivational Interviewing” techniques to influence patient behavioral changes to increase the level of compliance with customized care
    • Ensure documentation is accurate and comprehensive based on the standards of practice and current organization policies
    • Collaborate with colleagues, clients, care providers, customers and clinical providers to ensure continuity and efficiency as the patient is managed along the continuum of care
    • Ensure urgent situations are escalated and addressed appropriately

Experience/Education/Computer Proficiency Requirements:

    • Advanced computer skills such as virtual research, Microsoft Office, Tableau
    • Minimum of (5) years broad clinical experience
    • Minimum of (5) years of experience in home care, care coordination and transitions of care, case/disease management, telehealth or related specialty fields which may include Pulmonary, Endocrine, Cardiac, and other populations with chronic multiple morbidities, EMR implementation projects
    • Nursing degree from an accredited college or university
    • Flexible work hours, must be able to work in time zone specific to client requirements

Licensure and /or Certification Requirements:

    • RN licensure in appropriate state(s); Must be active and in good standing

Physical Demands & Work Environment:

    • Home office that is conducive to a productive workday; must be private, quiet and secure; free of distractions
    • Internet access must meet minimum requirements
