As Extended Primary Care, our clinicians work alongside the patient's physician between visits, with the record and care plan shared with the practice. With Accountable Primary Care, we are the patient's primary care provider and the outcomes are ours. Partners choose the accountability we hold.

We contract on outcomes, not activity

Where we are the accountable provider, there is no implementation fee, no program fee, and no base compensation. We agree on the savings method, benchmarks, and control group before signing; we validate savings in the partner's claims rather than our dashboard; and we pay our fees from that number and nothing else.
